Complete this form to submit an application to become a member of the Peretz Centre or to renew your membership.

Registration Policy

• Fees must be paid at the time of registration or at the office. Payment plans are available upon request.

• Our policy remains that no one will be turned away for lack of funds. If you require further accommodations, please contact Donna Becker at dbecker@peretz-centre.org.

• Membership fees over $50 may be eligible for charitable tax receipts.

• Membership fees apply to the current Peretz Centre fiscal year (Sept. 1 2024 to Aug. 31 2025).

• Membership applications are subject to approval by the Board of Directors. Our doors are open to people of diverse cultures, beliefs, genders, and sexual orientation.

• Members are automatically subscribed to the Peretz Centre community update (sent via email bi-weekly) and can unsubscribe at any time.

• By submitting this registration form, all individuals listed agree to uphold the constitution and comply with the bylaws of the Society.

• By submitting this registration form, all indivduals listed agree to the Peretz Centre Code of Conduct.

The Peretz Centre complies with the provisions of the British Columbia Personal Information Protection Act. A copy of our policy will be provided upon request. The Peretz Centre shall not use any personal information provided to it other than for the purpose for which that information was collected and will not disclose personal information to any third party without the knowledge or consent of the individual concerned.
